Making hollow blocks

I plan to make my own hollow blocks, I want good quality blocks which I am told should be no more than 40 per bag of cement. How does that equate to the ratio of cement to sand when mixing. I have seen figures of cement to sand of between 1:4 and 1:6, what should I use? I have also seen blocks that are very open in structure that contain gravel, is this done to reduce the cost? Is it a good idea to include some gravel? I have also seen references to blocks containing perlite (styrafoam grains) to make them lighter and give some insulation qualities, has anyone any experience with these?
